Machen niedrige Harnsäurewerte krank? Der Versuch einer Klärung anhand der verfügbaren wissenschaftlichen Literatur am Beispiel kognitiver Beeinträchtigung und Demenz.

Die Arbeit befasst sich mit der Frage: gibt es einen Zusammenhang zwischen niedrigen Harnsäurewerten und der Entwicklung von kognitiven Beeinträchtigungen oder Demenz?
Immer wieder wird in den Medien darüber berichtet, dass niedrige Harnsäurewerte das Risiko z.B. für die Entwicklung einer Demenz erhöhen könnten, und in der Folge setzen Patienten ihre harnsäuresenkende Medikation ab.
Nun weiß man aber inzwischen, dass hohe Harnsäurespiegel nicht nur zu Gichtanfällen und Harnsäuretophi führen können, sondern dass hohe Harnsäure ein eigenständiger Risikofaktor für Atherosklerose, Niereninsuffizienz, Fettleber oder Herzrhythmusstörungen - um nur das Wichtigste zu nennen - ist, und dass eine Normalisierung der Harnsäurespiegel diese Risiken deutlich reduziert.
Da diese Behandlung bedeutsam ist, ist es wichtig, die Frage zu klären, ob es einen wissenschaftlichen Beleg gibt, dass niedrige Harnsäurewerte andere Krankheiten wie etwa Demenz verursachen können.Um die Frage zu klären, wurden 41 0riginalarbeiten und 28 Sekundäranalysen ausgewertet, die sich in ihrer Methodik und in ihren Ergebnissen deutlich unterschieden.
23 0riginalarbeiten und vier Sekundäranalysen fanden einen Zusammenhang zwischen niedrigen Harnsäurewerten und kognitiver Beeinträchtigung bzw. Demenz.
Hingegen fanden sechs Originalarbeiten und neun Sekundäranalysen eine Assoziation zwischen höheren Harnsäurewerten und kognitiver Beeinträchtigung oder Demenz.
10 Sekundäranalysen und eine Originalarbeit konnten einen Zusammenhang zwischen höheren Harnsäurewerten und einer besseren kognitiven Leistungsfähigkeit finden, während sechs Originalarbeiten und sechs Sekundäranalysen keinen Zusammenhang fanden.
Die verfügbaren Publikationen zum Thema sind so grundsätzlich unterschiedlich, dass ein sinnvoller Vergleich der einzelnen Studien kaum möglich ist. Es fand sich nicht eine Studie, die die Frage eines Zusammenhangs zwischen Höhe des Harnsäurespiegels und der Entwicklung psychiatrischer Erkrankungen als zentrales Studienziel hatte.
In der Zusammenschau der verfügbaren Studienergebnisse ergibt sich folgendes Ergebnis:
Die verschiedenen Studien kommen zu unterschiedlichen, teils divergierenden, Ergebnissen. Es gibt keinen wissenschaftlichen Konsens dafür, dass niedrige Harnsäurespiegel das Risiko für die Entwicklung von kognitiven Beeinträchtigungen oder Demenz erhöhen.


Risks of Sexual Activity in Elderly and/or Sick People: Review

Study objectives: The review aimed to provide information about risk of sexual activity in elderly and/or sick patients. Is there condition which reduces or increases the risk of sexuality under these circumstances? 

Methods: Extensive search in medical databases PubMed, Medline, Embase, Science Direct and Google Scholar including multimodal search terms on the topic. 

Results: In the autopsy studies, coital deaths were <1% and <2%, respectively. Concomitant circumstances, such as partner relationship, spatial environment, or additional stress factors, were found to be important. ECG monitoring studies during sexual activity showed that arrhythmias occurred in a maximum of 56% of patients and cardiac ischemia in 50% related to sexual activity. ECG changes during daily activities showed comparable percentages. Fatal cardiac events were not observed. Physical fitness (≥3x/week physical activity/≥6 METs) showed a decreased relative risk of myocardial infarction related to sexual activity compared with less physically active patients (≤1x/week physical activity). A lowered odds ratio for fatal coronary heart disease was observed in patients with regular sexual contacts (≥2x/week) compared with patients with lower sexual contacts (< 1x/month). 

Conclusion: Coital deaths should be considered as rare events. In patients with coronary artery disease, arrhythmias during sexual activity were not significantly higher than during other activities of daily living, work, or exercise. Minimization of risk factors includes sexual activity with a steady partner (wife/husband), in a familiar environment without prior excessive alcohol consumption. Physical fitness and an active sexual life before cardiovascular events turned out to be positive factors.


What Contribution Can Doctors Trained in Romania Bring in Order to Remedy the Medical Deficit in Germany? Comparison of Medical Studies and Specialist Training between Romania, Germany, and Poland - Internal Medicine, General Medicine, and Anesthesia.

Aim: The growing shortage of doctors in Germany is a subject that is currently well documented in the mass media. The fields with the biggest deficits are internal medicine, general medicine, and anesthesia. One of the measures to correct this problem is employing doctors who have studied abroad. Theobjective of this study is to assess the extent to which the deficit of trained medics in Germany can be amended with doctors trained in Romania and Poland. Subject and methods: A very good overview of this phenomenon was acquired by putting together a  compilation of the educational process which leads to graduating as a medical doctor in Romania, Poland and Germany, the available literature regarding doctors migration and the annual statistical data provided by the German Medical Association. The study included processing the data obtained through two self-made questionnaires addressed to Romanian doctors. The first, with 129 respondents, was addressed to physicians who studied in Romania and were already working in Germany. The second, with 59 respondents, was addressed to Romanian students who wish to come and work in Germany. Results: The results of the Romanian survey were compared with similar studies carried out in Poland, revealing great similarities between the selected groups in training, motivation, way of thinking and perceptions of working conditions. The resulting graphics and figures provide a visual representation of the phenomenon, strengthening the conclusion. Discussion and conclusion: Taking into account the aging population in Germany, the increasing numbers of females in the medical profession, the growing trend of part-time work, the difficult medicine admission process and the unabated emigration of German doctors to the US, UK, Austria, and Switzerland, it is safe to conclude that Germany needs foreign-trained doctors. It became evident that not all the information regarding the training process in the three studied countries completely overlaps. There were also discrepancies between what is documented and the actual numbers of completed procedures that are required in order to complete the training. When taking all of this into account, the question this paper set to answer was positively answered, proof of which is the growing percentage of Polish and Romanian trained doctors working in Germany.


Side effects and interactions of the xanthine oxidase inhibitor Febuxostat

The paper addresses the safety of febuxostat and summarizes reports on side effects and interactions of febuxostat published by the cut-off date (last day of literature search) of 20 March 2018. Publications on side effects and the interactions of febuxostat were considered. Information concerning the occurrence of side effects and interactions in association with the treatment with febuxostat was collected and summarized in the review. The incidence of severe side effects was much less frequent than mild side effects (1.2–3.8% to 20.1–38.7%). The rate and range of febuxostat side effects are low at doses of up to 120 mg and only increase with a daily dose of over 120 mg. The publications reveal no age-dependent increase in side effects for febuxostat. In patients with impaired renal function, no increase in adverse events is described with a dose of up to 120 mg of febuxostat per day. Patients with impaired liver function had no elevated risk for severe side effects. A known allopurinol intolerance increases the risk of skin reactions during treatment with febuxostat by a factor of 3.6. No correlation between treatment with febuxostat and agranulocytosis has been confirmed. Possible interactions with very few medications (principally azathioprine) are known for febuxostat. Febuxostat is well tolerated and a modern and safe alternative to allopurinol therapy.


Comparison of the Specialist Medical Training in Internal Medicine between Germany, Austria and Switzerland - An Overview

Specialists for internal medicine are very important group of the medical profession. Although they make up the largest group of specialized physicians, there is a shortage of physicians, and in particular of internists in Germany, Austria and under some aspects in Switzerland. Germany, Austria and Switzerland show also an intensive transfer of physicians. It is therefore of interest to investigate if the term “internist” in the three countries under consideration is based on the same or equivalent education, in particular regarding the quality standards. Exchange date between the three countries, the organisation of the medical profession, the access requirements to the specialist medical training, the structure of the specialist’s training as well as the requirements for keeping the specialist’s name have been compared. The main differences are the access requirements and the linkage of the qualification to the independent exercise of the medical profession. Also regarding the performance catalogues, the Swiss education follows a different approach as in Germany and Austria.


Methylphenidate Misuse in Adults: Survey of 414 Primary Care Physicians in Germany and Comparison with the Literature

Objectives: The paper deals with the methylphenidate (MPH) misuse by adults in Germany. Results of a survey among primary care physicians/internists have been supplemented by a comparison with the literature. Methods: In the period from October 5-20, 2015, a survey was sent to 414 primary care physicians/internists in four German cities (n = 10 were undeliverable). The response rate was 58% (n = 235). 34 original works on MPH abuse worldwide were found in the literature and are used in the analysis of the present data situation. The literature published before November 9, 2015 is considered in this paper. Results: 14% of the doctors who took part in the survey said that they had been asked for MPH prescriptions without any medical indication. The most frequent reason given for the request (42%) was adult attention deficit/hyperactivity disorder (ADHD) not verified by documents. According to the comparison with the literature, university students had a lifetime prevalence of MPH misuse ranging from 0.8% to 16.6% and school children had a misuse rate of 4.0%. In the civilian US population, the misuse rate was 4.2%. Among patients in possession of a current MPH prescription due to a diagnosis, the lifetime prevalence was 29% and among adolescents with suspected alcohol and/or drug problems 20%. Conclusions: The survey of primary care physicians/internists as well as the analysis of world-wide studies indicates that the issue of methylphenidate misuse is a major problem and cannot be limited to the discipline of psychiatry. On the contrary, it affects other medical fields. MPH-prescriptions for children should also consider the option that adults and former patients with an intake of methylphenidate may misuse the substance. This has not yet been adequately investigated.


Effectiveness of a Four-Week Diet Regimen, Exercise and Psychological Intervention for Weight Loss

Introduction: Obesity is accompanied by restriction in the quality of life and an increased risk of morbidity and mortality. Cardiovascular, orthopedic, and metabolic disorders are among the possible consequences. In the management of obesity, a combination therapy that includes dietary, exercise, and behaviour modules has proven its worth. Aim: To evaluate the effect of weight-associated parameters, circulation associated parameters, glucose metabolism, body composition and life quality changes within a four-week inpatient rehabilitation program. Materials and Methods: Fifty-two patients underwent a 4-week inpatient rehabilitation program consisting of nutrition therapy, behavioural therapy and exercise therapy modules at the Eleonoren Clinic of Winterkasten, Germany. Results: The mean weight reduction of 52 obese patients - 40 (76.9%) males, 12 (23.1%) females; mean age 46 years; mean Body Mass Index (BMI) 43,79 kg/m² - achieved was 7.1 kg (from 1.20 kg to 17.50 kg), and the BMI reduction was 2.3 kg/m² (from 0.40 kg/m² to 5.40 kg/m²). The excessive weight loss was highly significant (p<0.001). Weight reduction was accompanied by an improvement in the diabetic metabolic state (lowering of fasting blood-glucose 20 mg/dl, postprandial blood glucose 26 mg/dl, HbA1c 0.27%). In all 73% of the patients suffered from arterial hypertonia. The significant mean decline of systolic and diastolic blood pressure was 12.8 mmHg and 6.8 mmHg, respectively. The resting pulse was reduced by an average of 11 beats per minute. The Bioelectric Impedance Analysis (BIA) revealed a significant reduction of body fat content (p<0.001). The subjective impression of impaired life quality (SF-36 questionnaire) improved significantly. Conclusion: The study clearly shows that the inpatient rehabilitation program at the Eleonoren Clinic was suitable to enhance the physical and mental state of people with obesity. In a two-year follow-up program the patients should take care of a permanent lifestyle change toward an improved dietary, movement, and health behaviour.


Vergleich der verschiedenen psychotherapeutisch tätigen Berufsgruppen bezüglich ihrer Zugangsvoraussetzung, Ausbildung, Legitimation, Vergütung und Haftung

Die Analyse von Gesetzen, Veröffentlichungen und Stellungnahmen ergab, dass sich die psychotherapeutischen Berufsgruppen stark unterscheiden. Es konkurrieren vier Ausbildungswege miteinander, deren Berufsbezeichnungen sich trotz Titelschutz stark ähneln. Die Ausbildungsvoraussetzungen differieren von „Hauptschulabschluss“ bis „Staatsexamen“. Die Ausbildungsdauer differiert von „keine vorgeschriebene Ausbildung“ bis „5 Jahre in Vollzeit“. Ausbildung und Prüfung differieren stark in Ausmaß und Inhalt. Entscheidende juristische Unterschiede finden sich bei den Themen Krankenhauseinweisung, Arbeitsunfähigkeitsbescheinigung, Rezept für Medikamente und Behandlung von Kindern und Jugendlichen. Heilpraktiker für Psychotherapie nehmen eine Sonderstellung ein. Sie verfügen weder über eine Approbation noch über eine strafgerichtliche Schweigepflicht. Eine Befragung im privaten Umfeld ergab, dass bei Laien und Fachleuten große Unwissenheit beim Thema „Psychotherapie-Ausübende“ besteht. Die Menschen sollten besser über das Berufsfeld „Psychotherapie“ informiert werden. Weiterhin bedarf es der Novellierung des Psychotherapeutengesetzes sowie des Heilpraktikergesetzes sowie einer Erweiterung des Titelschutzes.


Nicht mehr im Metabolischen Syndrom, aber von wachsender klinischer Bedeutung: Ist die Hyperurikämie Symptom einer Systemerkrankung?

Aktuelle wissenschaftliche Daten zeigen auf, dass Hyperurikämie möglicherweise nicht nur mit dem Risiko von Gichtanfällen und/oder Tophi verbunden ist, sondern das Symptom einer genetisch bedingten Fehlfunktion des Enzyms Xanthinoxidase sein könnte. Das Enzym Xanthinoxidase katalysiert den Abbau von Hypoxanthin zu Xanthin und Xanthin zu Harnsäure. Die dabei entstehenden Hyperoxidanionen bewirken oxidativen Stress mit Zellschädigungen. Hohe Aktivitäten von Xanthinoxidase in der Gefässwand führten zu der Hypothese, dass eine Überaktivität der Xanthinoxidase eine kausale Rolle bei der Entstehung von Atherosklerose spielen könnte. Dies könnte erklären, weshalb Patienten mit Hyperurikämie vermehrt kardiovaskuläre Erkrankungen, Erektile Dysfunktion und Niereninsuffizienz bekommen. Von den harnsäuresenkenden Medikamenten hemmen nur die Xanthinoxidasehemmer auch das Enzym Xanthinoxidase und damit dessen wohl atherogene Wirkung, und hier wohl Febuxostat deutlich stärker als Allopurinol. Der Artikel berichtet über die wichtigsten Aspekte von Gicht und Xanthinoxidase-Überaktivität und gibt Hinweise zur Therapie.

Recent scientific data show that hyperuricemia may be associated not only with the risk of gout attacks and/or tophi, but could be the symptom of a genetic malfunction of the enzyme xanthine oxidase. The enzyme xanthine oxidase catalyzes the degradation of hypoxanthine to xanthine and to uric acid. The resulting superoxide anions cause oxidative stress cell damage. High activity of xanthine oxidase in the vascular wall led to the hypothesis that overactivity of xanthine oxidase may play a causal role in the development of atherosclerosis. This could explain why patients with hyperuricemia increasingly develop cardiovascular disease, erectile dysfunction, and renal insufficiency. Of the urate lowering drugs only xanthine oxidase inhibitors also inhibit the enzyme xanthine oxidase, and thus its  atherogenic effect, and here probably Febuxostat is significantly stronger than allopurinol. The article reports on the main aspects of gout and xanthine overactivity and treatment.


Changes in behavior as side effects in methylphenidate treatment. Review of the literature.

Background: Our review of the scientific literature focused on an analysis of studies describing instances of methylphenidate treatment leading (or not) to behavioral changes in the pediatric, adolescent, and adult populations. Materials and methods: We conducted a literature search in PubMed, Medline, and Google using the keywords “methylphenidate”, “behavioral changes”, “adverse effects”, and “side effects”. A total of 44 studies were identified as reporting on the effects and adverse effects of methylphenidate administration, and were included in the analysis. Results: Five studies specifically set out to study, record, and discuss changes in behavior. Eight studies did not set out to study behavioral effects, but record and discuss them. A total of 28 studies recorded behavioral effects, but failed to discuss these further. Three studies did not include behavioral effects. Conclusion: This review records what data have been published in respect of changes in behavior in association with the use of methylphenidate. While there is some evidence to suggest that methylphenidate causes changes in behavior, the majority of the studies reviewed paid little or no attention to this issue. Based on the available data, it is impossible to determine the point at which such behavioral effects occur. The frequency of occurrence of behavioral effects is also impossible to determine with certainty. Based on the available data, it is not possible to rule out whether behavioral effects may persist or not persist once treatment is discontinued. In conclusion, despite countless publications and extensive administration, especially to children, we have insufficient data to judge the long-term effects and risks of methylphenidate taking.


Comparison of Medical Education between Germany and Poland Considering Internal Medicine, General Medicine, and Anaesthesia

With the continuing shortage of physicians in Germany, healthcare is increasingly being sustained by foreign physicians. The paper addresses the comparability of medical education curricula in Germany and Poland and the qualification of physicians having Polish exams and specialist medical training for clinics and practices in Germany. Methods: The Medical Licensing Act and the Model Specialty Training Regulation of the German Medical Association were compared to the respective Polish regulations for medical education. In orienting surveys, physicians with Polish exams were questioned and they provided information regarding their current situation in German healthcare. Results: Based on educational criteria, physicians with Polish exams and specialist training are well suited to work in medicine in Germany. They receive theoretical education comparable to that of German physicians; practical education during specialist medical training is more extensive in Germany. The survey could yield that Germany, as a job market, is very appealing for physicians with Polish exams. The primary reasons for this are better opportunities for training as well as better compensation. Conclusions: The solicitation of physicians equates to an economic loss for Poland. Nevertheless, the solicited physicians benefit from working in Germany. The European Union lacks common, detailed European standards for medical education.

The effects of nutritional juice supplementation on the extent of climacteric symptoms – an observational study

Objective: This study aims to evaluate the effect of daily dietary nutritional supplement on somatic, psychological and urogenital symptoms in postmenopausal women. Material and Methods: In this study 28 healthy, symptomatic, peri- and postmenopausal women of 47-67 years of age were allocated to use the nutritional supplement JuicePLUS+®. Primary research parameters: Menopause Rating Scale (MRS) was used to assess menopausal symptoms at baseline and after 8 and 16 weeks of treatment. Secondary parameters: Proliferation behaviour of vaginal smear was scored at baseline and after treatment. Results: Treatment with the supplement resulted in a reduction of somatic, psychological and urogenital symptoms. The overall MRS score showed an average improvement of 44.01%. Most benefits were observed for the psychological symptoms irritability (60.55%) and physical and mental exhaustion (49.08%), modest effects were observed for hot flashes (44.86%) and sleeping problems (35.56%). There was a minor improvement in sexual problems, 6 women reported an increased libido. No statistically significant effect was found in vaginal dryness and proliferation behaviour of vaginal mucosa. No adverse effects were observed. Conclusion: Dietary nutritional supplement may constitute an effective alternative therapy to conventional alternative medicine for somatic, psychological and sexual symptoms.


The Impact of a Pet, in This Case a Dog, on Physical Activity, Independence, Social Contacts, Health and Quality of Life of Elderly People

In a rural area near Munich, 52 persons aged 65 years and older were interviewed in person about health and social life circumstances. Findings: Dog owners take significantly longer walks than people who do not own dogs, averaging more than 120 minutes daily (p < 0.0001). On average, people who do not own a dog walked 38 minutes, cat owners 17 minutes. 94% of the dog owners were active daily compared to 14% of those who do not own a dog. Dog owners go to the doctor on average 5.3 times per year, people who do not own a dog 7.8 times. There was no difference between the two groups in regard to the use of medication. 80% would not move to a residential home for the elderly because of the dog, and 87% would want to take the dog along. 50% of the dog owners and 41% of those who do not own a dog had on average more than 3 contacts with other people, 6% of the dog owners and 23% of those who do not own a dog had less than one contact per day. 19% of the dog owners and 36% of those who do not own a dog would describe themselves as reserved in dealings with people. 71% stated that the dog had been very useful to help them overcome the loss of a person close to them. The W-5 [Wellbeing Five score] for dog owners was an average value of 75%, for those who did not own a dog 71%. Depression was uni- formly distributed in both groups. The BMIs in both groups were similar. There was no correlation to the length of the daily walk.  


Prüfzeiten des Einheitlichen Bewertungsmaßstabes: Wissenschaftliche Basis oder Schätzung? Eine Analyse am Beispiel der Koloskopie

Einleitung: Im EBM werden neben den Vergütungsziffern auch sog. Prüfzeiten angegeben. Diese nutzen die kassenärztlichen Vereinigungen für Plausibilitätsprüfungen. Wird eine bestimmte Zeitsumme überschritten, ist die Abrechnung auffällig und wird einer weiteren Prüfung unterzogen. Diese Untersuchung kann weitreichende Konsequenzen haben von Regressverfahren bis zur Anklage wegen Betruges. Wir haben deshalb untersucht, ob diese Prüfzeiten eine hinreichende wissenschaftliche Basis haben und am Beispiel der Koloskopie durch Analyse von Studien sowie eigenen Untersuchungsdaten überprüft, inwieweit die Koloskopieprüfzeit valide ist.
Ergebnisse: 1. Die Prüfzeiten wurden normativ und ohne wissenschaftliche Basis festgelegt. 2. In der wissenschaftlichen Literatur zur Koloskopie fanden sich überwiegend Untersuchungszeiten unter der Prüfzeit des EBM. 3. Bei den 1262 Koloskopien der eigenen Patienten ergab sich bei 74 % eine Prozedurzeit unter der vorgegebenen Prüfzeit, bei 26 % oberhalb.
Zusammenfassung: Es wird deshalb empfohlen, bis zur Validierung der Prüfzeiten des EBM durch wissenschaftliche Studien, diese im Rahmen der Plausibilitätsprüfung nicht mehr zu verwenden.


Visiting Patients at Home: Reasons and Efficiency of Dental House Calls

This retrospective study analyzes indications for house calls by dentists in a domiciliary setting. Its database consists in 69 visits made by a dentist office in Munich from 1998 to 2013 at the homes of 29 non-institutionalized seniors (≥62 years old, 15 females, 14 males, average age 83.7 years old). The analysis focused on the patients’ general state of health, their dental and oral health, the kinds of dental treatments administered, as well as economic aspects and the quantitative development in the frequency of house calls during the study period. The most frequent preexisting conditions that made a house call necessary were cardiovascular illnesses (60%) and dementia diseases (36%). Multi-morbidity, dependency on care, accessibility issues, and frailty because of advanced age played a central role. The patients’ oral health was acceptable, with the exception of those with dementia diseases, who were more frequently toothless (40% versus 5.6%) and had a higher percentage of carious and decayed teeth (26.3% versus 10.3%). The most frequent procedures were treatments for denture sores and the repair of removable dentures (53.6%). The main focus was on reparative dentistry. The average house call took 37 minutes including travel time. Thus, the time needed was a multiple of the time necessary for treatment in the dental office. The lack of economic efficiency may be the reason why many dentists in Germany hesitate to make house calls. Despite the introduction of additional fees for the treatment of patients in need of long-term care on April 1, 2013, the number of house calls is low and increases only slowly. At the same time, the need for house calls will rise in the future due to an increasing proportion of elderly persons in the population and a decline in the percentage of toothless elderly. 


Macht Kontaktabbruch zu den leiblichen Eltern Kinder krank? Eine Analyse wissenschaftlicher Literatur

Jeden Tag werden weltweit Kinder von ihren Eltern getrennt, sei es durch Flucht, Krieg, Tod, Trennung der Eltern, gerichtliche oder behördliche Maßnahmen. Die Erfassung von Folgen solcher Traumata und ihre Bedeutung für die medizinische Versorgung der Betroffenen werden immer wichtiger.
Ziel der vorliegenden Literaturanalyse war es, zu klären, ob Kinder, die in ihrer Kindheit und Jugend Kontaktverlust zu ihren lebenden Eltern erleiden, über das aktuelle Trauma hinaus länger anhaltende gesundheitliche Folgen zeigen.
Ergebnisse: Es gibt nur wenige Studien, die sich mit der Frage gesundheitlicher Folgen von Kontaktabbruch zwischen Kindern und ihren lebenden leiblichen Eltern befassen. Die Studien ergeben:
Kontaktverlust zu leiblichen Eltern führt unabhängig vom Lebensalter des Kindes bei Beginn und der Dauer des Kontaktverlustes zu einem erhöhten Risiko für die Entwicklung von schweren Depressionen, Angststörungen, Panikstörungen, Alkoholabhängigkeit, Drogenmissbrauch, Drogenabhängigkeit und Phobien. Kontaktverlust zu Mutter bzw. Vater bewirken beide erhöhte Erkrankungsrisiken mit unterschiedlichen Risikokonstellationen.
Die Folgen können lebenslang anhalten. So führte z.B. Kontaktverlust zu einem Elternteil durch Trennung – nicht aber durch Tod - zu einem erhöhten Risiko, an einer Alkoholabhängigkeit zu erkranken. Am ausgeprägtesten war dies bei Kontaktverlust zur lebenden Mutter, hier war das Erkrankungsrisiko für Alkoholabhängigkeit signifikant erhöht (iHR = 4,70 mit p < 0,0001) mit einer errechneten Erholungszeit von 115,5 Jahren. Kontaktverlust zu lebenden Eltern wirkt sich nachweislich der wissenschaftlichen Studien deutlich stärker aus als Kontaktverlust aufgrund von Tod. Durch Kontaktverlust zu lebenden Eltern werden die betroffenen Kinder etwa doppelt so stark und dreimal so lang belastet, wie bei Kontaktverlust durch Tod.

Was macht Österreich besser? Ergebnisse einer Befragung von medizinischen Sachverständigen in Österreich und Vergleich mit einer Befragung medizinischer Sachverständiger in Deutschland

2013 haben Jordan und Gresser in Deutschland eine Umfrage zur Begutachtungsmedizin durchgeführt und 548 medizinische und psychologische Gutachter befragt (DS 2014, 41: 71-83). Von den Mitwirkenden an der Befragung wurden Vorschläge für eine Verbesserung des Gutachterwesens gemacht , die in Österreich teilweise Gesetz sind. Die vorliegende Studie sollte klären, ob eine analoge Befragung von medizinischen Gutachtern in Österreich (n=924) andere Ergebnisse ergibt als in Deutschland.
Ergebnis: Das Gutachterwesen in Österreich ist patientenfreundlicher als das Gutachterwesen in Deutschland. Die Chancen, einen Gutachter zu bekommen, sind in Österreich durch die öffentlichen Datenbanken deutlich besser als in Deutschland. Die Bearbeitungszeit der Gutachten ist in Österreich kürzer.Die unterschiedlichen Regelungen für das Begutachtungswesen zwischen Österreich und Deutschland führen dazu, dass die Gutachter in Österreich zu einem späteren Zeitpunkt ihrer beruflichen Laufbahn und mit mehr Berufserfahrung als Gutachter tätig werden als in Deutschland. Die Einflussnahme von Gerichten auf Gutachter ist in Österreich nach Angabe der mitwirkenden Gutachter geringer als in Deutschland, aber deutlich häufiger, als nur gelegentlich. Gutachter, die über erhaltene Tendenzsignale durch auftragserteilende Gerichte berichten, erwirtschaften häufiger über die Hälfte Ihrer Einnahmen aus Gutachtertätigkeit als Gutachter, die keine Tendenzsignale erhalten haben. Besonders deutlich ist dies in der Gruppe der Psychiater.
Die Vergütung der Gutachtertätigkeit wird in Österreich stärker bemängelt, als in Deutschland.
Die Untergruppe der Psychiater weicht erheblich von den anderen ärztlichen Gutachtern ab, in Österreich noch deutlicher als in Deutschland. Psychiater machen am meisten Gutachten, arbeiten am wenigsten im Auftrag von Privatpersonen oder Anwälten, haben häufiger eine wirtschaftliche Abhängigkeit von Gutachten, erhalten am häufigsten bei Gutachten vom auftragserteilenden Gericht Tendenzsignale und ihren Gutachten wird – wenn sie vom Ergebnis erfahren - zu 100% gefolgt.

Die Befragung zeigt, dass der Gutachter der zentrale Faktor für den Verfahrensausgang ist.

The Effects of Plant Substances on the Extent and Clinical Course of Treatment Resistant Chronic Periodontitis

Aim. Investigation of the effect of dietary supplementation with plant substances on the course of refractory chronic periodontitis in various risk groups.
Materials and Methods. 30 subjects with refractory periodontitis were given the plant extract JuicePLUS+® whilst otherwise continuing with unchanged oral hygiene and diet. The primary research parameters were plaque accretion (API), bleeding index (BOP) and probe depth. Secondary soft parameters were the effects of the dietary supplementation on oral and general well-being.
Results. We observed a 19.3% reduction in the BOP. In 90% of subjects the bleeding index showed improvement. 63% achieved a marked improvement in their inflammatory state within two months; no notable improvements were seen later in the trial. The periodontal pocket depths reduced by on average 0.164mm (upper jaw) and 0.292mm (lower jaw). The soft endpoints improved markedly. No differences in the efficacy of the supplement between individual risk groups were seen. No side effects of the supplement were reported. 10 months after the study, 70% of the subjects reported a positive effect during the study.
Conclusion. Dietary supplementation with JuicePLUS+® can favorably influence the extent and clinical course of refractory periodontitis and can positively stimulate the psyche.


Endoscopic Retrograde Cholangiopancreatography - Analysis of 543 Procedures at the Kreiskrankenhaus Schongau [Schongau Hospital] 2006- 2011 and Comparison with Literature

Methods. For the present study we retrospectively analyzed the complication rates of endoscopic papillotomies over a period of six years (from 2006 to 2011) in the gastroenterological department of the Kreiskrankenhaus Schongau [Schongau Hospital], part of a community hospital in Upper Bavaria. Overall 543 examinations were evaluated. Our data were then contrasted with international studies.
Results. With a success rate of 93.74%, the Schongau Hospital had significantly better results than presented by other groups. The success rate is specified in the literature from 88% (n = 176), 90% (n =1242) to 91.6% (n = 2553). Additionally, with regard to moderate or severe post-interventional pancreatitis, the Schongau Hospital is well below the figures given in the literature: there is a mild pancreatitis in 44.8% (n = 262), moderate in 43.8% (n = 256) and severe in 11.4% (n = 67) of all patients. In Schongau, a moderate post-interventional pancreatitis occurred in 0.37% (n = 2) and a severe case in 0.18% (n = 1) of all patients. Our data shows outstandingly low rates of bleeding, acute post-interventional cholangitis or perforation when compared to the relevant literature.
Conclusions. Although the Schongau Hospital is relatively small  (180 beds), it  achieved equal or even better results in terms of endoscopic sphincterotomy than major centers. This may be due to the experienced endoscopic staff, regular and intensive skill enhancement as well as the strict indication for endoscopic sphincterotomy. Our study shows that size does not matter as long as personal motivation and skill sets are excellent. Quality assessments should note this.


Uric Acid Levels in Southern Germany in 1989. A Comparison with Studies from 1962, 1971, and 1984

Since 1962 our group has performed four studies on uric acid values in blood donors in Southern Germany (Bavaria). Uric acid levels in men have increased over the years, from 4.86 mg/dl in 1962 to 6.00 mg/dl in 1971, 5.60 mg/dl in 1984, and 5.90 mg/dl in 1989. Women aged 51 to 60 years had significantly higher uric acid levels than those in the fourth decade. In women treated with oral contraceptives uric acid levels were significantly lower than in other women of the same age. 
Hypouricemia (uric acid levels ≤ 2,0 mg/dl) was observed in three women, none of whom had a history of medication.
Hyperuricemia exists when uric acid levels are ≥ 6.5 mg/dl. In 1989 2.6 % of the female blood donors and 28.6 % of the males were hyperuricemic, with an increased risk of gout, nephrolithiasis, and nephropathy.


Battle against Aging and Folds: Benefit and Risks of the Semi-Permanent Fillers Polylactic Acid and Calcium Hydroxylapatite. Review

Background. Semi-permanent fillers are among the most favorable fillers on the market. Through their unique mode of action and its associated lasting aesthetic effect, they take an exceptional position.
Objective. Comparison of the two semi-permanent fillers polylactic acid and calcium hydroxylapatite in reference to the aesthetic result, patient satisfaction and side effects.
Method. Studies on side effects, patient satisfaction and aesthetic results after augmentation with semi-permanent fillers were analyzed.
Results. Semi-permanent fillers seem excellently suited for the augmentation of very deep wrinkles particularly in the lower half of the face. In general, high patient satisfaction can be determined with both fillers. Here, the effect from the polylactic acid can be verified for up to two years while no effect could be verified already after one year in a majority of the patients augmented with CaHA. Short terms side effects such as bleedings or erythema in the region of the augmented area have been observed in both fillers during augmentation. The incidence of nodules and granulomas seems significantly higher in augmentations with PLA compared to CaHA. Rare side effects such as an embolization of a blood vessel caused by the implant has been described for both fillers in case reports.
Conclusion. Semi-permanent fillers are superbly suited for wrinkle augmentation. Which filler is the preferred one in what case depends strongly on the individual needs of the patient and the therapist's experience.


Wie unabhängig sind Gutachter? Ergebnisse einer Befragung unter 548 medizinischen und psychologischen Sachverständigen in Bayern 2013

Die Zahl der in den letzten Jahren öffentlich gewordenen Justizfehler, für jedermann nachzulesen, wie z.B. im DER SPIEGEL 33/2011 vom 15.08.2011 über den Fall von Horst Arnold, die sich zum Teil zu Justizskandalen auswuchsen, hat das Vertrauen der Bürger in den Rechtsstaat erschüttert. Oft wurde weder den Opfern angemessen Gerechtigkeit zuteil, noch wurden die Ursachen der Justizfehler den deutschen Gesetzbüchern entsprechend rechtsstaatlich gewürdigt. In solchem Kontext öffentlich gewordene Fälle sind zum Beispiel die von Gustl Ferdinand Mollath, Ulvi Kulac, Horst Arnold, Jörg Kachelmann oder der des Vaters aus Memmingen, wobei jeder Fall eine ganz eigene Geschichte ist. Es wäre für die Wirksamkeit des Rechtsstaates wichtig, Fehler zuzugeben und Falschbeschuldigungen gemäß § 164 StGB bzw. § 239 StGB  zur Rechenschaft zu ziehen. So wurde beispielsweise im Falle von Horst Arnold die gerichtlich festgestellte Falschbeschuldigerin Heidi K. wegen der „Freiheitsberaubung in mittelbarer Täterschaft“ verurteilt, nicht aber wegen der Falschbeschuldigung – diese war bei Beginn der Ermittlungen verjährt. Auch im Fall des sieben Jahre lang unschuldig inhaftierten Vaters aus Memmingen wurde die Falschbeschuldigung wegen Verjährung nicht geahndet.

Verjährung ohne Klärung bahnt sich im Fall von Jörg Kachelmann an. Hier wurde wohl noch nicht einmal mit den Ermittlungen wegen des Verdachtes auf Falschbeschuldigung begonnen – die im Raum stehende Falschbeschuldigung wird wohl im Mai 2014 verjähren, die mittelbare Freiheitsberaubung wohl im Juli 2015. Und dies, obwohl das LG Mannheim mit Entscheidung vom 17.10.2013 festgestellt hat, dass die Herrn Kachelmann beschuldigende Frau Claudia D. keinen Unterlassungsanspruch hat, wenn Herr Kachelmann sie öffentlich als „Falschbeschuldigerin“ bezeichnet, da bereits die Strafkammer des LG Mannheim festgestellt habe, dass Frau D. „nachweislich [...] falsche Angaben gemacht“ habe.

Es geschehen Fehler, aber es fehlt offensichtlich an der Analyse und Beseitigung der Fehlerursachen. Man vermisst eine angemessene Fehlerkultur.

Eine zentrale und nicht selten auch mitursächliche Rolle bei den bekannt gewordenen Fehlurteilen des Rechtssystems spielen medizinische und psychologische Gutachten.

„Der Gutachter muß ein unabhängiges Beweismittel sein und bleiben [und] [...] darf von keiner der am Strafprozeß beteiligten Personen abhängig sein oder werden“. Doch gerade bei diesem von Rasch als „das heißeste Problem in der ganzen Gutachtenerstattung“ bezeichneten Prozess der Sachverständigenbeauftragung spielen in der öffentlichen Diskussion immer wieder Hinweise auf mögliche Einflussnahme der Gerichte auf die von ihnen bestellten Gerichtsgutachter eine Rolle.

Um einen Anhaltspunkt dafür zu erhalten, ob es Einflussnahmen auf Gutachter bei von einem Gericht in Auftrag gegebenen Gutachten gibt, und was die Gutachter selbst dazu sagen, wurde eine Befragung unter Gutachtern durchgeführt.

Der deutschlandweit fortschreitende Ärztemangel ist aktuelles Thema zahlreicher gesundheitspolitischer Diskussionen. Die Publikation befasst sich mit der Versorgungssituation und ärztlichen Wanderungs-bewegungen im ambulanten und stationären Bereich unter besonderer Berücksichtigung von Zuwanderung ausländischer Ärzte nach Deutschland sowie deren Konsequenzen.

Die Jahresstatistiken der Bayerischen Landesärztekammer und der Bundesärztekammer wurden in Bezug auf ausländische Ärzte und ärztliche Migration untersucht. Über die ambulante Versorgungslage Bayerns gibt die Analyse unveröffentlichter Fallzahlen und Planungsblätter der Kassenärztlichen Vereinigung Bayerns Aufschluss. Der stationäre Bereich ist mit einer orientierenden Befragung bayerischer Krankenhäuser erfasst und erschließt die aktuelle ärztliche Personalsituation.

Ergebnisse: Der Trend zur Emigration deutscher Ärztinnen und Ärzte vor allem in die Schweiz, Großbritannien, USA und Österreich ist ungebrochen. Die ambulante Versorgung für Bayern entspricht im fach- bzw. hausärztlichen Bereich noch der Regel- bzw. Überversorgung. Die Umfrage konnte dagegen für die stationäre Versorgung bestätigen, dass bei einem erheblichen Teil der befragten bayerischen Krankenhäuser ein Ärztemangel auftritt und bestehende Personalengpässe vielerorts nur durch die Beschäftigung von ausländischen Ärzten trotz erheblicher Sprachdefizite ausgeglichen werden können.

Um dem Ärztemangel langfristig zu begegnen, sind grundlegende Bedarfsplanungsreformen in Verbindung mit strukturellen Verbesserungen und Maßnahmen zur sprachlichen Integration ausländischer Mediziner ein notwendiger Schritt in Richtung einer bedarfsgerechten Versorgung.

Der Fall „Mollath“ hat deutschlandweit eine vielfältige Diskussion aus-gelöst. Gustl Mollath wurde in Folge eines psychiatrischen Gutachtens als „für die Allgemeinheit ... gefährlich“ eingestuft und sieben Jahre in einer psychiatrischen Klinik untergebracht. Im August 2013 wurde die Wiederaufnahme des Verfahrens durch Beschluss des Oberlandes-gerichts Nürnberg veranlasst.

Dieser und weitere Fälle lassen die Öffentlichkeit über das Begut-achtungswesen nachdenken. Ein besonders sensibler Punkt dieser Diskussion ist die Frage, inwieweit Gutachten objektiv, unabhängig und neutral sind.

Um dieser Diskussion eine wissenschaftliche Grundlage zu geben, wurde im November 2013 eine Studie zum Thema Begutachtungsmedizin in Deutschland am Beispiel Bayern im Rahmen einer Dissertation an der LMU München durchgeführt. Diese wurde im Deutschen Ärzteblatt vom 07.02.2014 in Auszügen erstmals als Originalarbeit veröffentlicht. Im Zuge der Studie wurden an 583 über das Internet ermittelte medizinische und psychologische Gutachter in Bayern Fragebogen versandt. 35 Briefe kamen als unzustellbar zurück, 548 waren zustellbar. 252 Befragte beteiligten sich an der Umfrage. Hiervon fünf gaben an, nicht gutachterlich tätig zu sein, vier teilten mit, nicht an der Umfrage teilnehmen zu wollen. Dies entspricht einer Rücklaufquote von 46,0 %.

Effects and side-effects of fruit-, vegetable- and berry-powder in case of JuicePlus+®

In daily challenge of job, stress and private life, nutrition is of growing importance. Hecticness of daily routine gives people insufficient time for well-balanced nutrition and balanced intake of fruits and vegetables. So for people it appears reasonable to use nutrition supplements as compensation for feared malnutrition. There is a tremendous variety of products, confusing for consumers. When patients ask their doctors for advice whether to take or not to take nutrition supplements and want to get tips for better well-being and prophylaxis, the recommendations like more exercise and healthy nutrition are inconvenient, difficult to implement, and often not feasible. The ingestion of nutrition supplements seems to be easier, and better than doing nothing. This might be, because physicians know too little about nutrition supplements, especially about effects, side effects and ingredients. For most of the nutrition supplements the scientific knowledge is bad, the knowledge of physicians is worse, and the attention to nutrition supplements is worst. In about 50 percent of patient contacts, physicians do not ask the patient for supportive ingestions, which would be the very least, especially in the fact, that about 80 per cent of people state to use nutrition supplements.

Browsing the scientific databases shows only few peer - reviewed scientific publications about over-the-counter preparations. One of the American market leaders in nutrition supplements, the National Safety Associates GmbH (NSA) with its product line JuicePlus+®, is one of few companies offering nutrition supplements and being engaged in scientific assessment of these products.

In conclusion: The nutrition supplement JuicePlus+® by NSA is one of the best investigated food concentrates worldwide, no other preparation is able to show these number of Pubmed listed publications. There are reports about beneficial effects and potential side- effects. There is a risk of overdosage of some ingredients like Vitamin E, β- carotin, Vitamin C and folic acid in order to guidelines of the German Society of Nutrition. Our aim is not to present new studies but to present the actual data basis of JuicePlus+®. There is a need for proper scientific research of JuicePlus+® and similar preparations, because the substances are highly effective. 


Atorvastatin: Gold standard for prophylaxis of myocardinal ischemia and stroke

Aim of this study was to compare the clinical benefit - reduction of heart attacks, strokes or deaths - of the different statins applying the results of randomized controlled endpoint studies.

We analyzed 11 published randomized controlled endpoint studies statin-to-placebo looking for the cardiovasculoprotective benefit of the 5 statins (atorvastatin, fluvastatin, lovastatin, pravastatin, simvastatin) tested: AFCAPS/TexCAPS, ASCOT, CARE, FLORIDA, HPS, PROSPER, LIPID, LIPS, MIRACL, 4S, WOSCOPS. Results:

  1. Statins produced substantial benefit for the patients, reducing the rate of cardiovascular morbidity and mortality.
  2. This benefit was independent of the patient´s initial cholesterol or LDL-cholesterol concentrations and could also be demonstrated in patients who had average or low cholesterol levels.
  3. Men and women showed a comparable benefit from statin treatment, elderly patients a little more than younger patients.
  4. The statins did not have like effects. There were clear differences in potency as well as in the interval between initiation of treatment and the onset of clinical benefit.
  5. Estimating 5 years of treatment, cardiac morbidity decreased with atorvastatin up to 44 %, with pravastatin up to 36 %, with fluva- or simvastatin up to 32 % and with lovastatin up to 24 %, approximately.
  6. Estimating 5 years of treatment, morbidity of suffering from stroke decreased with atorvastatin up to 41 %, with simvastatin up to 34 % and with pravastatin up to 31 %, approximately. For fluva- and lovastatin there are no comparable data. Within the first 16 weeks of treatment following an acute coronary syndrome relative risk for suffering a non-lethal stroke was reduced with atorvastatin 80 mg/day up to 59 % compared to placebo, the relative risk for stroke up to 50 %.
  7. The fastest onset of clinical benefit - reduction of fatal and non-fatal cardiovascular events, hospitalization and necessity of invasive interventions - was demonstrated by treatment with atorvastatin (rapid, within some weeks), followed by lovastatin (after one year), fluva-, prava- and simvastatin (after 1½ - 2 years).
  8. These results were achieved with atorvastatin 10 mg/day (80 mg/day used in MIRACL), lovastatin 20 to 40 mg/day (caused by dosage titration), pravastatin 40 mg/day, simvastatin 20 to 80 mg/day (caused by dosage titration) or fluvastatin 80 mg/day.
  9. The advantage of atorvastatin may be due to its ability to reduce cardiovascular disease by stopping the growth of plaques in artery walls.

Atorvastatin was the most powerful compound in the group of statins, improving patients´ health and expectation of life.

The authors of the studies agree, that patients at risk for cardiovascular diseases should be treated with a statin irrespective of initial cholesterol concentrations, sex or age. If an acute cardiovascular event has happened, statin treatment should be initiated early to improve the prognosis of these patients at high risk, independent from initial LDL cholesterol values.

Summing-up of these 11 trials, the best results and the greatest benefit for the patients were achieved with atorvastatin, which might be considered to be the gold standard for prophylaxis of cardiac ischemia and stroke.

Etwa 30 Prozent der Männer und 3 Prozent der Frauen haben erhöhte Harnsäurewerte, und jeder zehnte Patient mit Hyperurikämie entwickelt eine Gicht. Bei der Therapie des akuten Gichtanfalles sind Steroide oder nichtsteroidale Antiphlogistika Mittel der Wahl. Für die Vermeidung einer chronischen Gicht ist eine harnsäuresenkende Dauertherapie notwendig. Mittel der Wahl sind die Urikostatika Allopurinol oder Febuxostat (seit 2010 in Europa zugelassen, in diesem Artikel noch nicht erfaßt), alternativ das Urikosurikum Benzbromaron. Bei sachgerechter Therapie der Gicht unterscheiden sich Lebensqualität, Arbeitsfähigkeit und Lebenslänge dieser Patienten nicht von der von Gesunden.


Erectile Dysfunction: Comparison of efficacy and side effects of the PDE-5 inhibitors Sildenafil, Vardenafil and Tadalafil - Review of the literature

Since introduction of the PDE-5 inhibitor sildenafil 4 years ago, there has been a fundamental change in the treatment of erectile dysfunction (ED). Intracavernosal or intraurethral injections of vasoactive substances or penile implants as mechanical aids now play hardly any part in it. The development of the PDE-5 inhibitors vardenafil and tadalafil prompts the question of whether and how these three substances differ in terms of their efficacy and adverse effects. Sildenafil has proven to be a very effective medicinal product. Studies with a follow-up period of up to 6 years have been conducted. The success rate of sildenafil varies in the group of ED patients with an organic underlying disease from 43% in patients who have undergone radical prostatectomy to 85% in patients with a neurological underlying disease, and amounts to an average 82% (range 43-85%, 100mg). In an evaluation of spontaneous reports of deaths associated with sildenafil, the FDA concluded that there was no deducible evidence of an increase in the mortality rate among sildenafil users compared to the general population. In fact, fewer deaths associated in time with the ingestion of sildenafil were reported than might have been expected purely statistically on the basis of the normal mortality rate for men in this age group. According to the initial studies conducted, vardenafil and tadalafil demonstrate efficacy data approximately comparable to those of sildenafil. As yet, insufficient data are available to evaluate the adverse effects of vardenafil and tadalafil, particularly their long-term use and use in high-risk groups. Sildenafil has already been used by over 20 million men in over 110 countries and is one of the beststudied pharmacological substances available. This adventage in terms of knowledge and safety data makes sildenafil a safe and reliable treatment for patients with erectile dysfunction.


Amoxicillin-Clavulansäure als mögliche Ursache schwerer Lebererkrankungen.

Berichte über Lebererkrankungen in Zusammenhang mit Amoxicillin-Clavulansäure (A-C) belegen, dass A-C neben leichten Befindlichkeitsstörungen auch ernste Nebenwirkungen verursachen kann. Bei 153 Fällen lag nach Konsensuskriterien ein Zusammenhang zwischen A-C und einer Lebererkrankung vor. Die mittlere Behandlungsdauer betrug 13,9 Tage, die Zeit bis zum Auftreten hepatischer Symptome 3,6 Wochen, bis zur Normalisierung der erhöhten Leberenzyme 11,5 Wochen nach Therapiebeginn; drei der 153 Patienten überlebten die Erkrankung nicht. A-C kann, wenn auch selten, schwere bis tödliche Nebenwirkungen verursachen, unabhängig von Alter oder Geschlecht des Patienten. Vermehrt gefährdet sind Patienten mit umfangreicher Begleitmedikation. Bei lokalisierten oder unkomplizierten Infektionen ist abzuwägen, ob A-C erforderlich ist. Bei Therapie mit A-C sollten innerhalb der ersten beiden sowie vier Wochen nach Therapiebeginn die Leberparameter untersucht werden, um eine mögliche arzneimittelbedingte Lebererkrankung frühzeitig zu erkennen.


Amoxicillin-clavulanic acid therapy may be associated with severe side effects - Review of the literature

There is a growing body of evidence that amoxicillin-clavulanic acid may induce severe adverse effects in patients.

A medline search of case reports and reviews on amoxicillin-clavulanic acid induced adverse effects was performed. The criteria of a consensus conference on the reporting of drug-induced liver disease were applied.

Amoxicillin-clavulanic acid has been associated with drug-induced cholestatic hepatitis in 208 reported patients. In 153 evaluable patients there were 106 males and 47 females with a mean age of 60 years (1-90). Liver associated co-morbidity and co-medication does not play a major part in the development of disease. In most instances respiratory tract infection and sinusitis were treated by amoxicillin-clavulanic acid with a mean treatment duration of 13.9 days and a reaction time until first onset of jaundice of 25.2 days average. Infection and cholestasis from other reason were ruled out in most patients. Liver injury was classified according to laboratory parameters to be hepatocellular in 35 patients, cholestatic in 24 patients and mixed in 83 patients. Normalization of liver enzymes was observed 11.5 weeks after onset of drug administration (average); three of 153 patients did not survive the adverse event.

Amoxicillin-clavulanic acid which is marketed for treatment of respiratory infections and sinusitis/ otitis may in some cases induce severe adverse effects and death in patients of different age, especially if they are on multidrug regimens. In consideration of this fact many authors recommend to reflect carefully, whether amoxicillin-clavulanic acid is necessary in treatment of patients with localized or uncomplicated infections. If amoxicillin-clavulanic acid is prescribed, transaminase, alkaline phosphatase and bilirubin tests should be obtained within the first two weeks and after four to five weeks after beginning of treatment to recognize early enough undesired hepatic side effects.

Unter den stoffwechselbedingten Arthritiden ist die Gicht die häufigste. Etwa 3% der Männer und 0,3% der Frauen erleiden im Laufe ihres Lebens eine Gicht, und es gibt in praktisch jeder Arztpraxis Patienten mit Gicht. Typisch für die Gicht ist die hochakute, extrem schmerzhafte Monarthritis im Wechsel mit beschwerdefreien Intervallen. Ursache ist eine Hyperurikämie, die meist durch eine familiär gehäuft auftretende Ausscheidungsschwäche für Harnsäure oder - selten - durch einen Enzymdefekt bedingt ist. Patienten mit Hyperurikämie aufgrund einer Erkrankung außerhalb des Purinstoffwechsels (z.B. Niereninsuffizienz) oder einer Therapie (z.B. mit Zytostatika) entwickeln weniger häufig Gelenkentzündungen. Diagnostik und Therapie der Gicht sind einfach und kostengünstig. Die Dauertherapie ist unkompliziert, wirksam und gut verträglich. Bei sachgerechter Therapie der Gicht unterscheiden sich Lebensqualität, Arbeitsfähigkeit und Lebenslänge von Gichtpatienten nicht von Gesunden. Unbehandelt führt die Gicht zu schweren Schäden an den Gelenken und zum Verlust von Lebensqualität und Arbeitsfähigkeit bis zur krankheitsbedingten Berentung.
